Families and Communities Together

The Families and Communities Together (FCT) program at Community Healthlink provides both Intensive Care Coordination (ICC) and Family Support and Training (Family Partners). 

Intensive Care Coordination uses the Wraparound Care Planning Process to coordinate multiple services and supports for youth with Serious Emotional Disturbance (SED). Involvement and participation in ICC is voluntary. A care coordinator is responsible for working with the youth and family to do safety planning and learn about the youth’s and family’s strengths, culture, and needs. The care coordinator then helps the family assemble a Care Planning Team which will meet regularly to develop an Individualized Care Plan.

Intensive Care Coordination facilitates care planning and coordination of services for MassHealth youth, with serious emotional disturbance (SED), under the age of 21, and enrolled in MassHealth Standard or CommonHealth who meet the medical necessity criteria for this service.

Family Support and Training is provided by Family Partners.  Family Partners are caregivers who have experience raising a youth with special needs. Family Partners can help parents to

• Better navigate the child-serving systems

• Identify available services and supports in the community

• Develop connections with self-help or support groups

• Support and coach the parent/caregiver in reaching the youth’s and family’s goals

With our partners at LUK, Inc., we serve as the Community Service Agency (CSA) for the Worcester East, Worcester West, and North Central Areas.

FCT Worcester West: Barre, Brookfield, East Brookfield, Hardwick, Holden, Hubbardston, Leicester, New Braintree, North Brookfield, Oakham, Paxton, Princeton, Rutland, Spencer, Warren, West Brookfield, and Worcester
